Job Description:

This salaried position requires a current RN license and home care, rehabilitation, or public health experience. I. Position: Care Coordinator, Personal Assistance Services (RN), (Direct Service Exempt)

II. Reports To: Personal Assistance Services Manager

III. Position Summary: To provide timely and efficient services to the PAS program by visiting consumer homes and assessing and developing appropriate plans of care: act as liaison to physician, consumer and agency: and provide direction to and evaluate performance of personal care workers.

IV. Essential Functions of the Job:

  1. Assess the need for service using tools designated by WI Medicaid. Refer consumers to other services as appropriate. Assessment to include the member’s environment (social and physical), functional level and pertinent cultural factors and community supports.
  2. Secure written orders from the recipient’s physician. These orders are to be renewed annually unless the physician or payment source specifies otherwise.
  3. Provide payment source with necessary documentation within specified deadlines to approve and maintain service.
  4. Maintain proper medical and service records. Monitor time and service reporting mechanisms for PCW’s and instruct on their use.
  5. Develop a plan of care for the consumer giving full consideration to the consumer’s preferences for service arrangements and choice of personal care workers. Include a copy of the care plan in the consumer’s service record, and review the plan at least every 60 days, updating as necessary.
  6. Supervise the PCW according to a written care plan and at least every 50-60 days provide a supervisory review of PCW services in the member’s home and provide ongoing training needs and evaluation of competencies.
  7. Maintain documentation related to the supervision of PCWs- to include intermittent face to face contact between supervisor and PCW, review of daily timesheets, annual performance evaluations and documentation of training of delegated medically oriented tasks.
  8. Attend training sessions and conferences as needed.
  9. Coordinate schedules and services for consumers in accordance with Independent Living Philosophy of consumer controlled care with Consumer Service Representatives.
  10. Be available for on-call services on a rotating basis.
  11. Provide advocacy and support for the Independent Living Philosophy to all PAS staff and consumers

Experience and Skills:

IV. Qualifications:

  1. RN degree and current Wisconsin license to practice nursing.
  2. At least one year of supervisory or administrative experience in personal care services or in a related program.
  3. Experience in home health, rehabilitation, public health, and /or independent living.
  4. Good written and verbal communication skills.
  5. Experience with/knowledge of Medicaid system.
  6. Transportationally mobile or able to arrange own transportation.
  7. Knowledge of federal, state and local laws related to community based health care.
  8. Ability to conduct home visits in environments that may or may not be accessible.
  9. Preferred bilingual ability (including English and Spanish, Hmong, or Arabic fluency).
